HealthPathways portal

For health professionals
Directed towards local GPs, nurses, allied health professionals and specialists, Western Sydney HealthPathways is a partnership initiative between WentWest, Western Sydney Local Health District (WSLHD) and the Sydney Children’s Hospitals Network (SCHN). A web-based information portal, HealthPathways offers western Sydney health professionals, locally relevant and clinically developed pathways for an extensive range of health conditions to support in the management of care and referral to services.

Healthy Western Sydney portal

For local health care consumers & carers
Similar to HealthPathways, Healthy Western Sydney is a web-based resource portal for western Sydney health care consumers and their carers. Healthy Western Sydney offers locally-tailored and evidence-based information and resources on a diverse range of health conditions including: healthy living; men’s and women’s health; children's health; older person’s health; mental health and more.

LinkedEHR

LinkedEHR is a care planning system specifically designed to provide a Shared Care Plan. It records goals, targets and referrals so that patients will receive the right level of care required for their chronic disease conditions. LinkedEHR supports the management of patients with chronic conditions, enabling a team of health professionals to support the coordination and delivery of care.

My Health Record

An initiative of the federal government, My Health Record is a secure online summary of your health information. You can control what goes into it, and who is allowed to access it. You can choose to share your health information with your doctors, hospitals and other healthcare providers.
