Improving diabetes and obesity outcomes in Western Sydney requires a shared and coordinated effort across both primary and secondary healthcare. Leaders from across the health system recently came together to attend a half‑day Metabolic Health Planning Workshop held at Nepean Clinical School.
WentWest, Western Sydney Primary Health Network, was proud to work alongside our colleagues from Central Eastern Sydney PHN, Nepean Blue Mountains PHN and Sydney North PHN, in partnership with Sydney Health Partners’ Diabetes and Obesity Clinical Academic Group (DO‑CAG). An inspiring Consumer panel shared valuable lived‑experience insights with clinicians in how to support more coordinated and person‑centred care for people living with metabolic conditions.
Attended by WentWest Director, Jane Assange, discussions focused on shared priorities for integrated, patient‑centred care and system‑wide planning.
Officially opened by the General Manager of Nepean Hospital, Mr Brad Ceely, the session reinforced the critical role of strong collaboration with general practice in delivering sustainable metabolic health outcomes for our diverse communities.
