Supporting Health Professionals
The core of what we do is to help community members access the right care at the right time. As a Primary Health Network, we commission local services and programs to help provide that access with many services available in partnership with health professional consultations.
Due to developments with COVID-19 and recent Government announcements regarding social distancing and unnecessary travel, we are currently working remotely. To support health professionals in their role as a primary care provider, we also offer training and education opportunities within Western Sydney and help them access clinical pathways, health resources, referrals and information on programs and priorities for our commissioned services, partner programs and government initiatives.
Collaborative Commissioning involves patient centred co-commissioning groups identifying and prioritising local health needs and developing care pathways to improve patient and community outcomes. It aims to address the gaps in patient care and embed local accountability to ensure care is truly integrated for patients.
