On Wednesday 12 October, Pen CS announced the 2022 Pen CS Awards Winners at the Australian Association of Practice Management (AAPM) National Conference, which celebrates data-driven quality improvement in general practice and Aboriginal Medical Services around Australia.
The 2022 Pen CS Awards received nearly 100 nominations, and three of our Western Sydney practices were celebrated for their achievement: Lejeune Family Medical Practice won the Digital Innovation Award, Rosedale Medical Practice won the Data Driven Clinical Excellence Award and Crestwood Family Practice was a finalist for the Patient Centred Care Award.
Lejeune Family Medical Practice

The Digital Innovation Award recognises an organisation that has exhibited an innovative use of technology to drive quality improvement programs in their general practice. This year, Lejeune Family Medical Practice won the Digital Innovation Award, having previously won the Data Driven Improvement Award in 2021.
Lejeune Family Medical Practice actively screened patients at risk of atrial fibrillation and cardiovascular disease and worked with a multi-disciplinary team using Heart Bug, and Topbar to flag patients under the Cardiology in Community program. Cardiology in Community is a Western Sydney Care Collective initiative, run in partnership between the Western Sydney Primary Health Network and Western Sydney Local Health District. Between January and August 2022, Lejeune completed 178 Healthy Heart Checks, compared to 142 in 2021. Utilising innovative technologies has helped Lejeune improve their cardiac services and improved team management for the patient.

Rosedale Medical Practice

The Data-driven Clinical Excellence Award recognises an organisation that has shown exceptional clinical outcomes, by translating data insights into actionable activities. The Data-driven Clinical Excellence Award Winner was Rosedale Medical Practice.
Rosedale Medical Practice devised a simple and effective diabetes strategy with limited resources and in a financially sustainable way. Using the CAT PIPQI dashboard and Lumos report, the multidisciplinary team significantly improved outcomes. Patient and care team experience improved, billing and practice sustainability had improved, and patients reported increased satisfaction from a proactive team-based approach with an increase to 80%, from 50%, in the recording of the Hba1c for patients with diabetes at Rosedale Medical Practice.

Crestwood Family Practice

The Patient Centred Care Award recognises an organisation that has demonstrated exceptional patient centred care. Crestwood Family Practice was a finalist for this award.
Pen CS’s suite of programs has helped Crestwood Family Practice manage chronic conditions and immunisation. The technology was pivotal in identifying 100 patients to be screened and educated for cardiovascular disease, out of which 40 patients were enrolled in the Cardiology in Community program, within the last 12 months. Pen CS has also been instrumental in the recall of 113 patients aged 71-79 years for the Zoster vaccine, which resulted in 71 vaccinations.
Monthly reports from Pen CS led to the recall of patients at risk of developing diabetes, those living with diabetes outside of the target ranges, and those overdue for their Diabetes Cycle of Care checks. Crestwood Family Practice helped these patients maintain ongoing visits with their health care professionals to create preventative strategies.
