These Terms and Conditions outline the requirements for participating in WentWest’s Needs Survey Campaign 2026 – Apple Watch Incentive Draw. By submitting a completed survey participants agree to the following:

Terms and Conditions

  1. Promoter

The promoter is WentWest Limited, trading as Western Sydney Primary Health Network (WSPHN).

Research provider: Outcome Services (engaged by WentWest to administer the survey and prize draw process separately from survey responses).

  1. Eligibility

To be eligible, entrants must:

  • Work within the Western Sydney Primary Health Network (WSPHN) region.
  • Complete one (1) of the four WentWest Needs Surveys (General Practice; Allied Health; Residential Aged Care Homes; Commissioned Service Providers).
  • Provide valid contact details (name, email and phone) in the prize‑draw section.
  • Be 18 years or over.
  • Enter once only (strict 1 entry per person).

Ineligible: Employees of WentWest and Outcome Services and their immediate families.

  1. Survey Period
  • Opens: 23 February 2026
  • Closes: 7 April 2026 

Late entries will not be accepted.

  1. Prizes

There are five (5) Apple Watches to be won across the four surveys:

  • General Practice Survey: 2 x Apple Watch
  • Allied Health Professionals Survey: 1 x Apple Watch
  • Residential Aged Care Homes Survey: 1 x Apple Watch
  • Commissioned Service Providers Survey: 1 x Apple Watch

Total prize pool value: AUD $2,000.

Prizes are not transferable, not exchangeable, and not redeemable for cash. Apple Watch phone model to be advised once purchased.

  1. How to Enter

Eligible participants enter by:

  1. Completing an eligible survey during the Survey Period; and
  2. Providing their contact details in the prize‑draw entry section.

Only one (1) entry per person will be accepted (multiple or automated entries are invalid).

  1. Privacy & Data Handling

WentWest and Outcome Services will ensure survey confidentiality and data minimisation:

  • Survey responses remain anonymous unless the participant explicitly opts to link their comments to their name within the survey.
  • Prize‑draw contact details are collected and stored separately by Outcome Services and cannot be linked to survey responses.
  • Outcome Services will use a separate list of names solely to conduct the draw.
  • Only the winning entrants’ contact details will be retained for prize fulfilment; all other prize‑draw contact details are deleted after the draw.
  • WentWest will receive the names and contact details of winners only to arrange prize delivery.
    Privacy is handled in accordance with WentWest’s and Outcome Services’ applicable privacy policies and Australian privacy law.
  1. Draw & Winner Selection
  • Draw method: A random number generator will be used to select winners from the eligible prize‑draw list maintained by Outcome Services.
  • Draw timing: 9 April, 12:00pm AEDT
  • Draw location/administrator: Outcome Services 

Category allocation: One winner will be drawn per category listed in Section 4 (2 x GP; 1 x AHP; 1 x RACH; 1 x CSP).

If a drawn entry is invalid or ineligible, another entry will be drawn.

  1. Winner Notification

Winners will be notified by email and/or phone within 3–5 business days of the draw. 
If a winner cannot be contacted or does not claim the prize within 3 business days, the prize may be redrawn using the same method. 

  1. Prize Delivery & Publicity

WentWest will coordinate prize delivery or collection with each winner.
With consent, WentWest may arrange a photo opportunity and may publish the winners’ first name, role/organisation, and suburb for promotional purposes.

  1. Conditions of Entry

By entering, participants agree:

  • They meet the Eligibility criteria (Section 2).
  • Their contact details may be used solely for administering the prize draw and prize fulfilment.
  • WentWest and Outcome Services may disqualify entries that are incomplete, ineligible, or appear to involve manipulation or multiple submissions.
  1. Liability

To the extent permitted by law, WentWest and Outcome Services are not responsible for:

  • any technical malfunction, communications failure or platform outage affecting survey completion or entry;
  • any loss, damage, or injury (including indirect or consequential loss) suffered in connection with the promotion or any prize.

Nothing in these T\&Cs limits statutory guarantees under the Australian Consumer Law.

  1. Variations & Cancellation

WentWest may amend these T\&Cs or cancel the promotion if it cannot be conducted as planned due to circumstances beyond its control. Any material changes will be communicated via the survey hub.

  1. Acceptance of Terms

Participation in the prize draw constitutes acceptance of these Terms & Conditions.
