We are excited to announce we have launched Healthy Western Sydney, a free service directory for community members and health professionals in Western Sydney. The directory currently offers information on aged care, dementia and palliative care services in Western Sydney and we plan to expand to include other services in the future.

Information available in the directory is tailored to both community and health professionals. As a health professional, you can access specific toolkits and resources that enable you to provide better levels of care for your patients, for example in assessing and caring for those approaching end of life.

You can also use the directory in conversation with your patients, their family and carers who may be looking for services such as financial support, transport services, home care mobility and carer support networks.

The directory also includes links to additional helpful resources including the National Health Services Directory, Provider Connect Australia and Western Sydney HealthPathways.

Information in the directory has purposely been structured in an easy to read, user friendly way, so that you receive a snapshot of the services offered, including information such as what mode of delivery the services is provided in (i.e. in person, online, over the phone or in-home), whether there is a cost involved, if a referral is needed and/or if a booking is required.

We have also included audience information, especially useful if you are looking for services specifically for First Nations or Culturally and Linguistically Diverse (CALD) patients.
